If you have Medicare, a commercial PPO plan, or are self-pay, you may select a new primary care provider of your choosing. We recommend contacting your insurance company directly for a list of in-network providers or using their online directory to identify a provider who best meets your needs. Should you require assistance with transferring your medical records, our office will be happy to support you. Your medical records will be maintained and made available in accordance with applicable state and federal regulations.
